Delta RM-15 Apnoea Monitor

The respiration monitor developed by Delta Medical International provides simple, robust and reliable apnoea monitoring for neonates and young babies. The apnoea monitor is lightweight and portable, making it suitable for use in the full range of hospital settings – NICU, maternity, post-natal and paediatric departments. The device uses a disposable pneumatic sensor for reliability and convenience.

Apnoea in pre-term babies may be caused by an underlying illness but is most commonly due to idiopathic apnoea of prematurity (AoP). Incidence in pre-term infants varies from around 10% in those born at over 34 weeks’ gestation to over 50% at 31 weeks and occurs with the vast majority of babies delivered at less than 29 weeks1,2.

Central and Obstructive apnoea account for around 40% and 10% of episodes respectively, with the balance of cases being Mixed apnoea1,2. Central apnoea emanates from the central nervous system, causing cessation of respiratory effort and resulting in an absence of chest wall movement. Obstructive apnoea is caused by airway occlusion and resulting in absence of airflow despite ongoing respiratory effort.

Mixed apnoea is found in approximately half of all interrupted respiratory events, with mixed aetiology; central deficit is usually preceded by airway obstruction, but occasionally arises first2.

AoP typically arises 2-7 days after birth, with earlier onset indicative of other causes. Babies are at increased risk of developing central nervous system problems that may be associated with apnoea if they suffer from certain complications at birth, or a range of conditions thereafter2,3. A few examples include:

  • Birth trauma
  • Acute or chronic pain
  • Thermoregulation problems
  • Infections (e.g. sepsis)
  • Neurological issues
  • Cardiovascular complications
  • Gastrointestinal problems
  • Maternal drug dependency
  • Family Centred Care

The causal factors leading to apnoea invoke additional stress for the newborn infant’s development. Parents suffer anxiety as a result of respiration problems and it is important that they understand what is happening to their new baby. An awareness of how apnoea monitoring can contribute to improved outcomes can help to reduce their concern, contributing to the overall developmental care process.

Apnoea Detection

Apnoea can arise unexpectedly in infants at risk of respiratory failure and can easily go unnoticed. Whilst pulse oximetry will alert nursing staff to falling oxygen saturation, more timely intervention is possible if early detection of respiratory arrest is possible.

The consequences of cessation of breathing can be serious, or even catastrophic2,3,4. as a result a reliable method of warning clinical staff of breathing problems can be very valuable. In today’s busy neonatal care environment, the reassurance obtained from monitoring for apnoea cannot be underestimated.
